29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: a) On or about August 23, 2018 during machine setup of the seamer on canning line 3, the sensor eye on the machine malfunctioned and created a pinch point hazard from the air knife apparatus. Energy control procedures were not utilized prior to the incident to prevent employee exposure to the moving part hazard. b) On or about September 6, 2018 canning crew leads were clearing jammed cans from canning line 9 without utilizing lockout procedures and/or equivalent protections to prevent employee exposure to moving part hazards.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about August 23, 2018 employees were exposed to machine hazards associated with pinch point hazards from moving parts when they were engaged in servicing and maintenance activities on the Canning Line 3. The employer did not ensure that energy control application steps to control the electrical energy sources were implemented prior to allowing employees to start setup on the canning line. The machine was not shut down or turned off prior to engaging in this servicing and maintenance activity. b) Employees were exposed to machine hazards associated with pinch point hazards from moving parts when they were engaged in servicing and maintenance activities on the Canning Lines 1, 2, 4, 5, 9, 10, 11, 12, and 13. The employer did not ensure that energy control application steps to control the electrical energy sources were implemented prior to allowing employees to start setup on the canning lines. The machine was not shut down or turned off prior to engaging in this servicing and maintenance activity. c) On or about September 6, 2018 employees were exposed to machine hazards associated with pinch point and/or crushing hazards from moving parts when they were engaged in servicing and maintenance activities on the Canning Line #9. The employer did not ensure that energy control application steps to control the electrical energy sources were implemented prior to allowing employees to start clearing jams on the canning lines. The machine was not shut down or turned off prior to engaging in this service and maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device: On or about September 6, 2018 two Canning Crew Leads engaged in servicing and/or maintenance tasks did not each utilize energy control procedures and/or affix individual lockout devices to the energy disconnects.
