29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced: In the welding area; On or about September 5, 2018, the leads to the welding machine (SS# MG440673R) being used had damaged insulation which exposed the bare conductors. This condition exposed employees to electrical shock hazards.
