29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:     a. Mountain Avenue, Berkeley Heights, NJ (near Horseshoe Lane)     An employee lashing cable to utility poles was not protected from falls while working from an elevated work platform.  An employee, elevated in a truck mounted boom lift (aerial lift) did not attach to the basket or boom, a fall harness or body belt, when elevated to approximately 12 feet.    Violation observed on or about September 5, 2018
