29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     On or about September 9, 2018, at a worksite located at 425 Victoria, Youngstown, OH 44515 during commercial roofing work, employees were exposed to falls of approximately 26 feet.  The employees were using a ladder that was not extended 3 feet above the roof and no grasping device was provided to assist employees in mounting and dismounting the ladder.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    On or about September 9, 2018, at a worksite located at 425 Victoria, Youngstown, OH 44515 during commercial roofing work, employees were  exposed to falls of approximately 26 feet.  The employees were performing roof installation activities on a low slope commercial building roof with no fall protection system/device.    McNulty Roofing Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1187517, citation number 01, item number 003, and was affirmed as a final order on December 8, 2016, with respect to a workplace located at 6088 Smith Road, Brookpark, OH 44142.

The employer did not ensure employees were provided safety and health training or that there was a system in place to recognize and correct unsafe working conditions.
