29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about 6 September, 2018, in front of the building under construction, an employee working from an aerial lift did not utilize fall protection, to prevent him from falling out or being ejected from the basket approximately 16 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about September 6, 2018, in front of a building under construction, an employee working from the 2nd floor near the edge did not utilize fall protection, to prevent him from falling approximately 16 feet.
