29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about August 29, 2017, at a work-site located next to 6604 Hollis Blvd, Hudson, Ohio 44236; during the installation of trusses and framing material, the employer did not ensure employees working on the second floor while trusses were being flown into position were protected from possible danger of head injury from impact, or from falling, flying objects, and or falling debris from suspended loads of trusses above head.

29 CFR 1926.502(k)(1): The fall protection plan shall be prepared by a qualified person and developed specifically for the site where the leading edge work, precast concrete work, or residential construction work is being performed and the plan must be maintained up to date. On or about August 29, 2018, during the installation of wood framing material at a job site located at 6604 Hollis Blvd, Hudson, Ohio 44236; the employer did not ensure employees working on setting trusses on a single family home were protected from a fall of approximately 22 feet by utilizing a fall protection system. The Miller Bros. Building & Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1162153, citation number 01, item number 02 and was affirmed as a final order on 8/30/2018, with respect to a workplace located at Prestige Housing Development near Majestic Oaks Drive Lot 4, Hudson, OH 44236
