29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person:    (a) SCAFFOLD: The employer did not ensure that scaffolding placed into service was constructed according to the design of a qualified person, exposing employees to falls from elevations. 29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were modified in order to intermix them and a competent person did not determine that the resulting scaffold was structurally sound:    (a) SCAFFOLD: Modified scaffolding manufactured by different manufacturers, was placed into service without being inspected by a competent person to ensure it was structurally sound, exposing employees to falls from elevations.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.      (a) SCAFFOLD: Employees performing stucco operations from a fabricated frame scaffold did not use an acceptable means of access. An employee was observed climbing the scaffold exterior and the cross-bracing to access the scaffold.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:     (a) SCAFFOLD: Employees engaged in stucco operations worked from a supported scaffold which did not have toeboards erected along the edge of platforms to prevent employees on lower levels from being struck by overhead tools, materials or equipment.
