On 8/8/2018, an employee of Sandoval Construction, LLC was working on a residential building located at 2100 Pure Street, Saint Peters, MO.  Employees were installing the flooring of the second level approximately 14 above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building wearing fall harness without being connected to an anchor point.
