29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. (a) The aisles and pedestrian walkways inside the warehouse door that connects the warehouse and the garage, were not marked, therefore exposing pedestrians to being struck by forklifts.

29 CFR 1910.178(n)(1): All traffic regulations shall be observed, including authorized plant speed limits. A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times: (a) On or about August 31, 2018, an employee driving the Yale Electric Forklift #64, failed to keep it under control at all times.

29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing. (a) The view of forklift operators driving from the garage into the warehouse was obstructed, thereby exposing pedestrians inside the warehouse door, as well as forklifts driving the other way, to struck-by hazards.
