On or about 09/06/2018, at the job site, employees were not provided hard hats while roofing material was being thrown from the roof of a 3 story building into trash container that was being lifted by an aerial lift. The employees on the ground were picking up debris & trash directly under the area where the old roofing materials were being thrown into the container.  b.
