29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) On and before 9/10/2018, at housekeeping mixing room, for the housekeepers not wearing eye protection while using and prepping surface cleaners that are corrosive including but not limited to:  Fusion Green, Fusion Violet, and bleach.  The employer did not provide an eye protection.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident    (a) On and before 9/10/2018, at electrical panel room, for the maintenance employees working on resident room's electrical panel. The breakers in the main electrical panel were not label to indicate which electrical panels were covered by each breaker.
