29 CFR 1926.102(a)(1):  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a)  On or about September 7, 2018, employees were exposed eye injury in that the employees did not use safety glasses while working with or near pneumatic nail guns. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  (a)  On or about September 7, 2018, workers were exposed to shock and burn hazards from an extension cord that was use to power the air compressor.  The insulation of the yellow extension cord was pulled back from the plug placing strain on the live conductors.    Abatement Note:  Abatement certification is required for this item (see the enclosed "Certification of Corrective Action Worksheet").
