OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of fire and explosion: On or about August 23, 2018, at the McCollum A Unit 17H well site, employees were working at the well site, where equipment with ignition sources were operating within 50 feet of open top tanks emitting flammable vapors. This hazardous condition provided an ignition source as the released flammable materials from the tanks traveled downwind to the operating equipment, exposing employees to the hazard of fire and explosion.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about August 23, 2018, at the McCollum A Unit 17H well site, first and second shift employees were working at the mixing station for the coiling operation without the use of flame-resistant personal protective clothing (FRC), exposing employees to the hazards of being burned in the event of a flash fire and/or explosion.
