29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:      a) Grinding Department- On or about August 28, 2018, the employer did not ensure that electrical equipment was free of damage and recognized hazards, such as damage to the faceplate of a 110 Volt junction box for the  Besly Grinder Machine 78.    No abatement certification or documentation is required for this item.

29 CFR 1910.333(b)(2)(i): The employer did not maintain a written copy of the procedures outlined in paragraph (b)(2) of 29 CFR 1910.333 and did not make it available for inspection by employees and the Assistant Secretary of Labor and his or her authorized representatives:    a) Facility wide- On or about August 29, 2018, the employer did not maintain a copy of the written safety related work practice procedures outlined in 29 CFR 1910.333(b)(2) for employees working on or near exposed energized parts such as troubleshooting on equipment up to 110 Volts.          In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  a) Facility wide- On or about August 29, 2018, the employer did not ensure that electrical protective equipment was provided for employees performing troubleshooting tasks on or near exposed energized parts, such as electrical rated rubber gloves with leather over gloves to protect from electrical hazards.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of the incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on OSHAs public web site at www.osha.gov:    a) Grinding Department- On or about August 28, 2018, the employer did not report to OSHA, within twenty-four (24) hours, a work-related injury that occurred on August 28, 2018 that resulted in an inpatient hospitalization.     No abatement certification or documentation is required for this item.
