29 CFR 1910.25(d)(2): Have a maximum riser height of 9.5 inches (24 cm)    On or about September 11, 2018, employees were exposed to falling/tripping hazards caused by the use of a non-uniform riser with a riser height at approximately 12 inches.  The employer did not ensure a maximum riser height of 9.5 inches.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.28(b)(6)(i): The employer must ensure each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard.    On or about September 11, 2018, employees were exposed to crushing hazards while operating a crusher/trash compactor. Employees were required to operate the crusher/trash compactor while accessing a step riser.  The employer did not install a guard rail near the entrance of the crusher/trash compactor to protect employees from failing into the dangerous equipment.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.141(d)(1): Washing facilities were not maintained in a sanitary condition:      On or about September 11, 2018, employees were exposed to unsanitary conditions while being allowed to clean/wash their hands and other body parts inside restrooms located outside the production and office area. The employer did not ensure restrooms were maintained in a sanitary condition.          There is no abatement certification or documentation required for this item.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.     On or about September 11, 2018, an employee was being potentially exposed to chemical burns while handling chemicals such as, but not limited to, 1st Ayd #990 Aluminum & Stainless Steel Cleaner. The employer did not provided an adequate emergency eyewash and/or shower for quick drenching while cleaning with and/handling the corrosive chemicals.     There is no abatement certification or documentation required for this item.
