Boston, Wichita, Kansas:  Employees engaged in residential roofing  activities from a 6:12 pitch roof were exposed to fall hazards of approximately 11 feet from the eave of the residence to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the residence.
