29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    No hard hats were worn, or were available, for two employees working below and around the apartment building at 506 W. St. Louis St., De Soto, MO on 6-11-18, where hand tools and materials were used by employees working on the three story building installing new roofing. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Three employees installing roofing panels on the three story apartment building at 506 W. St. Louis St., De Soto, MO on 6-11-18, and who were wearing harnesses, but were not connected to lanyards that were available and were said to be anchored, had not received site specific training on how fall protection was to be made.  No fall protection training had been provided according to and covering the required elements of the standard, 1926.503(a)(2)(i) through (viii).  Abatement documentation is required for this violation.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:    During roofing work at 506 W. St. Louis St., De Soto, MO on 9-11-18, an employee climbed an extension ladder while holding an approximate 20 ft metal panel, in order to hand the panel to the employees on the roof.  The employee did not maintain three points of contact with the ladder at all times.
