29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Employees were exposed to amputation hazards.  The guard for the hold down clamp assemblies for the Cincinnati Shear was inadequate as the guarding was hinged and not secured. A portion of the guard was also absent on the far right side facing the Cincinnati Shear. The far left stationary guard did not drop down closer to the table next to where the hinged guard started. Both ends of the front guards did not wrap around the front side to prevent employees hands or fingers from getting into the sheer and holddown assemblies. On or about September 8, 2018, a temporary employee received an amputation injury from the Cincinnati Shear hold down.
