The interior door in close proximity to the west exit that could have been mistaken for an exit was not marked "Not an Exit."  This condition was noted on or about 9/5/2018.

29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  (a) Central aisle way between Suites A, B and C. The south emergency exit door was locked from the inside. This condition was noted on or about 9/5/2018.  US Department of Agriculture-NRCS was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.36(d)(1), which was contained in OSHA inspection number 1167403, Notice number 1, Item number 2, issued on 09/06/2016, and became a Final Order on 9/28/2016.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":  (a) Suite B. The west exit door was not marked "EXIT". This condition was noted on or about 9/5/2018.  The USDA-NRCS was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.37(b)(2), which was contained in OSHA inspection number 1261808, Notice number 1, Item number 1, issued on 11/17/2017, and became a Final Order on 12/12/2017 with respect to a workplace located at 808 Iowa Speedway Dr., Newton, IA  50208.  The USDA-NRCS was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(b)(2), which was contained in OSHA inspection number 1167403, Notice number 1, item number 5 and was affirmed as a final order on 9/28/2016 with respect to a workplace located at 1513 N. Ankeny Blvd., Suite 3, Ankeny, IA 50023.

The portable fire extinguisher in the vehicle with license plate A340390 did not have an annual maintenance check.  This condition was noted on or about 9/5/2018.
