On or about September 11, 2018, on the residential construction site where employees were exposed to a fall hazard greater than 6 feet when performing roofing work without a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about September 11, 2018, at the north side of the residential construction where employees were exposed to a fall hazard when using an extension ladder to access a roof without extending the ladder side rails at least three feet above the roof landing.
