29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: Employees engaged in working with a blue extension cord to provide power to a handheld drill on the north end of the building at 15301 Green Road, Olathe, Kansas were exposed to electrical hazards due to no grounding pin on the plug end of the blue extension cord. The handheld drill was being used in a hole in the ground against the building that contained water.
