29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) At Hwy 10 Bogue Creek Bridge, Lisman, Alabama: On or about August 31, 2018 and at times prior; the employer failed to ensure employees involved in pile driving activities were trained on the hazards associated with the job to include but not limited to proper procedures for setting and removing the metal bars that the hammer rests on exposing the employees to amputation hazards.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: a) At Tanner Construction, Company, Ellisville, Mississippi: On or about September 10, 2018 and at times prior; the employer exposed employees to chemical hazards to include but not limited to diesel fuel, unleaded fuel, oxygen and acetylene in that employees were not provided the new information and training on the UN Global Harmonized System (GHS) to include Safety Data Sheets (SDS).
