OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or about August 31, 2018 at a job site located at 8432 Lake Shore Drive, Chagrin Fall, Ohio 44022; an employee was exposed to a fall hazard of approximately 25 feet while working aloft in a Maple tree which was designated for removal. The employer did not ensure prior to beginning any tree removal operation, the chain saw operator, equipment operator, and/or crew leader carefully consider relevant factors pertaining to the tree and site to include tree decay to ensure a safe removal operation.
