29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On September 20, 2018, Christopher Adkinson dba Adkinson Construction was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:       a.    Citation 1 Item 1, and  b.    Citation 2 Item 1a, and c.    Citation 2 Item 1b, and d.    Citation 2 Item 2.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about September 11, 2018, employees performing roofing activities were exposed to fall hazards of approximately 10.5 feet and the employer did not ensure fall protection was used.

Christopher Adkinson DBA Adkinson Construction and Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1236437, citation number 1, item number 1a and was affirmed as a final order on July 20, 2017, with respect to a workplace located at 7322 Radnor Rd, Peoria, IL 61615.
