29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1) On or about September 13, 2018, the employer failed to protect employees from the moving and rotating parts of the mold which is inside of pit #1. Employees are exposed to these hazards during the cleaning process in between mold pours. 2) On or about September 13, 2018, the employer failed to protect employees from the moving and rotating parts of the mold which is inside of pit #2. Employees are exposed to these hazards during the cleaning process in between mold pours. 3) On or about September 13, 2018, the employer failed to protect employees from the moving and rotating parts of the mold which is inside of pit #3. 29 CFR 1910.179(j)(2): Frequent inspection. The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: On or about September 13, 2018, the employer failed to ensure that all components of the various overhead cranes and hoists, to include but not limited to the Yale 2 ton (serial #AH4391XB), were inspected prior to being operated. The employer has a generic inspection form that they use that has a schematic of the locations and types of cranes and hoists in the foundry but no check list identifying the components to check for functionality. Employees just mark a check by the crane and sign and date the form.
