29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Employees who perform setup activities on and around the United 272 Rotary Die Cutter Machine were not required to; (1) physically locate and operate the electrical and pneumatic energy isolating devices in such a manner as to isolate the machine from the energy sources. (2) affix lockout devices to each energy isolating device. (3) prior to starting work on the machine that had been locked out, verify that the isolation and deenergization of the machine had been accomplished. On September 5, 2018, at the United 272 Rotary Die Cutter Machine, a helper was exposed to an amputation hazard from the nip point between the drum and anvil roller while installing a cutter die on the drum when the cutter die slipped from the helper's grip of his hand and the rotating drum pulled the helper's left hand into the nip point do to a malfunctioning momentary contact switch.
