29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Building "O" & Pool House - On or before September 13, 2018 employees reroofing the buildings were wearing safety harnesses that were not attached to an anchor point to prevent them from falling off the roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Pool house - On or about September 13, 2018 employees reroofing the buildings used an extension ladder to get on the roof that did not extend 3 feet past the roofs edge and was not tied off.
