On 8/8/2018, at the job site of 2200 Lithonia Industrial Blvd., Lithonia, Georgia, employees were exposed to Ammonia exposure hazards as the employer did not develop and implement required safe work practices. Instances included:  a - The employer did not develop specific written Lockout/Tagout procedures for employees replacing a valve on a 2 in. HP liquid line on an active Ammonia refrigeration system July 4th, 2018 and for compressor service/maintenance in June, 2018.  b - The employer did not require or document Hot Work permits when employees performed welding operations to an active ammonia system July 4th, 2018.
