A) J Alexander Roofing of Findlay, LLC on or about September 13, 2018 at the address of 114 Palmer Avenue, Bowling Green, Ohio: The controlling employer did not ensure a sub-contractor and its employees were protected from falls while tearing off old roofing materials (asphalt shingles) of a residential type structure. The employees were exposed to a potential 12 - 21 foot falls to the level below.
