Employees were exposed to slipping hazard due to fluid from the bodies. The employer did not ensure the floors were kept clear from fluids.

29 CFR 1910.134(d)(3)(iii)(B)(2): The employer did not implement a change schedule for respirators not equipped with an End of Service Life Indicator ESLI that would ensure cartridges were changed before the end of their service life:       (a) On or about 07/17/2017, at facility , for the diener at the morgue whom wear half face scott respirators.  They use formaldehyde cartridges when they go in the morgue's cooler.  The employees did not know how often to change the cartridges. The employees were not provided a cartridge changeout schedule prior to being required to wear the respirators.

The employer did not implement procedure to prevent facial hair on respirator users.
