Upstairs Storage Room: Cardboard boxes, bags, and plastic sheets were on the floor within close proximity of walking/working area. B. Basement Furnace Room: Tools were leaning against wall and cardboard was on the floor. C. Maintenance Room: Tools were leaning against wall, and some of them were very close to the exit doors. D. Basement Storage Room: Cardboard boxes, a printer, and plastic sheets were on the floor within close proximity of walking/working area.

Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: Shipping/Receiving Area: The employer did not ensure that the up-lifted metal docking plate approximately 12 inches above from the walking/working surface was either barricaded/shielded or employees were wearing appropriate types of personal protective equipment (PPE) to prevent employees from contacting sharp edges and sustaining injuries.
