29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) 55-1/2 Main Street, back building, north side, on or about 9-14-18:  Two employees were working on a sloped roof and were not protected from falls, exposing the employees to a fall hazard of 25 feet from the gable end.     b) 55-1/2 Main Street, back building, north side, on or about 9-14-18:  Two employees were working on a sloped roof and were not protected from falls, exposing the employees to a fall hazard of 13feet 1 inch from the eave.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) 55-1/2 Main Street, back building, north side, on or about 9-14-18:  Two employees were using two portable aluminum ladders for roof access/egress, with side rails that were 42 inches below the landing surface, exposing the employees to a fall hazard 13 feet 1 inch (eave to ground) from the top of the 20 foot section of ladder.
