29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: JOB SITE: 683 Trade Center Boulevard, Chesterfield, MO 63005, on or about 9/14/2018, workers were observed on top of a flat commercial building roof, greater than 6 feet in elevation, actively installing a new roof, and were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. The workers were exposed to a fall hazard.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: JOB SITE: 683 Trade Center Boulevard, Chesterfield, MO 63005, on or about 9/14/2018, workers were observed on inside of a 40 yard dumpster, approximately 6 tall from top edge of dumpster to ground level, arranging roofing debris. The workers were not provided points of access where there was a break in elevation of 19 inches (48 cm) or more. The workers were exposed to a fall hazard.
