29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: JOB SITE: 500 Auto Mall Drive, O Fallon, MO 63368 on or about 9/14/2018, workers were observed on a JLG 460 SJ, S/N: 0300078479 aerial lift, approximately 11 feet 10 inches above ground level, actively washing and cleaning the metal trim on the building and a belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. The workers were exposed to fall hazard.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold (including aerial lifts) trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: JOB SITE: 500 Auto Mall Drive, O Fallon, MO 63368, on or about 9/14/2018, the employer did not have each employee who performed work while on a scaffold (including aerial lifts) trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The employer had the employee operate a JLG 460SJ, Unit #360004, S/N: 03000289479 without any form of fall protection, to include but not limited to a body belt with lanyard attached to the basket.
