29 CFR 1910.22(a)(1): All place of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitation condition:    WORKSHOP: The wooden step on which an employee was standing while operating the Supermax/Bridgeport milling machine (equipment) was not firmly secured to prevent from movement.     On or about September 13, 2018, one of gloves worn by an operator/employee was pulled into the rotating drill of the above equipment, because the wooden step suddenly jingled/displaced. The operator/employee sustained a finger amputation from the incident.
