29 CFR 1926.454(c): When the employer had reason to believe that an employee lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer did not retrain each such employee so that the requisite proficiency was regained: a) Job site, 4000 Weatherburn Drive, Valencia, PA: On or about September 14, 2018, during construction activities, the employer did not ensure that employees working on a scaffold were retrained where safety concerns arose regarding it's use. Deficiencies noted during the inspection included, but were not limited to, failing to ensure scaffold baseplates sat level on mudsills, working on a scaffold while still erecting it and permitting employees to climb the ends of walk through frame scaffold to get up/down from the scaffold.
