29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   As of February 28, 2019, the employer had not submitted to the agency documents demonstrating abatement was completed for each serious violation indicated in Citation 1 Item 1, and Citation 1 Item 2 from inspection 1346890, issued on December 12, 2018 with a final order date of February 11, 2019.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:  885 Riverdale Avenue, West Springfield, MA: On or about August 24, 2018 each employee on walking/working surfaces were not protected from falling through holes more than six feet above the next lower level, by personal fall arrest systems, covers or guard rails erected around such holes.  (a) Second Floor Elevator shaft, shaft was not guarded by a standard guard rail system which would withstand 200 lbs. of lateral thrust. (b) Pool room, in ground pool with step into depths of 60 inches was not guarded by standard guard rails or cover. (c) First floor elevator shaft was not guarded by standard guard rails and had step into depth of approximately 55 inches.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  885 Riverdale Avenue, West Springfield, MA: On or about August 24, 2018  the employer had not certified that fall protection training had been conducted by preparing a written record of such training to include the employees name or other identifier, signature of employee, date of training and signature of trainer.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
