The scaffold did not have a standard guardrail system that confirmed with OSHA standards. The 2x4 toprail on the front of the scaffold fell off from the vibration of it being lowered while it was being moved.  The 2x4 toprails and midrails on the sides of the scaffold were hung with thin wire. Toe boards were not present on the front or sides of the scaffold.
