29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) 151 Du Rhu Drive (Building 4 - East Exterior Wall Area (Long Side)): On or about September 17, 2018, the employer failed to instruct or train their employees in recognizing, eliminating or controlling hazards associated with work performed in a Genie S-80X Boom lift thus exposing their employees to hazards such as but not limited to catapult hazards of up to approximately fifty-five (55) feet to the concrete or ground below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) 151 Du Rhu Drive (Building 4 - East Exterior Wall Area (Long Side)): On or about September 17, 2018, the employer failed to ensure employees were using a body belt with lanyard attached to the approved anchor point while using an aerial lift (Model: Genie S-80X) to perform construction activities; exposing employees to catapult hazards and fall hazards of up to approximately fifty-five (55) feet to the concrete or ground below.
