29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about September 13, 2018 the employer did not ensure the point of operation where the battery becomes sealed on the heat seal machine on AGMCOS Line 14 was adequately guarded. An employee suffered an amputation injury during operation while removing a battery after it had been sealed.
