The employer reported the fatality on September 17, 2018, more than 8 hours after they became aware of the death.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Worksite, 2245 Ticonderoga Blvd, Chester Springs, PA - Dock Yard: On or about September 5, 2018 an employee was admitted to the hospital as an in-patient for treatment of a work-related incident upon being struck in the head/neck by falling cargo freight. The employer reported on the in-patient hospitalization on September 12, 2018.
