29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about September 13, 2018:  At the worksite located at 150 Bobo Trail in Canton, Georgia, employer failed to document and utilize specific procedures to control hazardous energies on all machinery located within the facility, exposing employees to cuts, lacerations, amputation and possibly death. 29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group.  On or about September 13, 2018:  At the worksite located at 150 Bobo Trail, Canton, Georgia employer failed to ensure employees were protected by means of group lockout hardware and devices when multiple people are preforming servicing or maintenance tasks, exposing employees to cuts, lacerations, amputation and possibly death.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

OR IN THE ALTERNATIVE    29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    On or about September 13, 2018:    At the worksite located at 150 Bobo Trial Canton, Georgia, employer failed to ensure that chains and sprockets were properly protected by guards on the rip saw, exposing employees to caught in hazards.
