29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group. On or about September 13, 2018: At the worksite located at 150 Bobo Trail, Canton, Georgia employer failed to ensure employees were protected by means of group lockout hardware and devices when multiple people are preforming servicing or maintenance tasks, exposing employees to cuts, lacerations, amputation and possibly death. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
