For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section:  a) Meadows Boulevard, Castle Rock CO, 80109: On or about September 19, 2018, employees were using hand-held power saws to cut concrete that were not equipped with integrated water delivery systems per the engineering and work practice controls specified in Table 1.  b) Meadows Boulevard, Castle Rock CO 80109: On or about September 19, 2018, employees were operating a Case skid steer with a concrete breaker attachment to demolish concrete sidewalks where the skid steer did not have a completely enclosed cab in that both sides were partially open and the cab was also not under positive pressure per the engineering work and practice controls specified in Table 1.  Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

The employer did not conduct an exposure assessment of employees exposed to respirable crystalline silica during this process.
