Maintenance Shop Bay 1, left -hand side, outer entrance, on or about 19 September 2018 and times prior thereto; an energized electrical wires were not guarded against accidental contact. An energized exterior wiring bundle was missing an approved enclosure. This condition exposed employee to electrical shock and electrical burn hazards.

Maintenance Shop, Bay 2, East wall; on or about 19 September 2018 and times prior thereto; two instances were noted where flexible cords were used in lieu of fixed wiring:         a)  A blue medium-duty extension cord, length unknown, was attached to the wall and routed to supply power to a AIWA stereo set, serial number unknown.         b)  A orange medium-duty extension cord was attached to the wall and routed to provide a power source to a Kenmore washing machine, serial number unknown.    This condition exposed employees to an electrical shock and electrical burns hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Maintenance Shop, Bay 2, workbench adjacent to stanchion; on or about 19 September 2018 and time prior there to; the employer did not ensure that portable cord and plug equipment was inspected prior to use. An energized orange 50-foot medium duty extension cord had damaged to the outer protective jack. A Mikata Model 90058, 5-inch Angle Grinder, Serial number 2340114A was plugged into the extension cord. This condition exposed employees to an electrical shock hazard.

Maintenance Shop, adjacent to Bay 2; on or about 19 September 2018 and times prior thereto, a 500-gallon flammable fuel tank was not protected from vehicular traffic. Fuel tank is located adjacent to a moderate vehicular traffic area. This tank supplies fuel for the hot water heater. This condition exposed employees to fire and explosion hazards.

Samson Tug and Barge Company, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(B), which was contained in OSHA inspection number 981120, citation number 1, item number 1 and was affirmed as a final order on 06 July 2014, with respect to a workplace located at Samson Tug and Barge Company, 3311 Tongass Ave. Ketchikan, AK.

Administrative Office; on or about 19 September 2018 and times prior thereto; the employer did not ensure that the Summary of Work-Related Injuries and Illnesses (OSHA Form 300A) were complete and accurate. On the 2017 OSHA Form 300A, the Total Recordable Cases (65) is different from Total of Injuries and Illnesses (9). The average number of employees (9) and total hours worked (191,152) exceeds total hours in a calendar year (8,760).This condition leads to inaccurate reporting of injuries and illnesses.
