29 CFR  1910.134(c)(1):  A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a)	West Bay Maintenance Shop - on or about October 22, 2020, employees performed welding operations and were required by the employer to wear tight fitting respirators without a written respiratory program including worksite specific procedures.  (b)	West Bay Maintenance Shop - on or about October 22, 2020, employees performed welding operations and were required by the employer to wear tight fitting respirators without medical clearance exposing them to physiological hazards.  (c)	West Bay Maintenance Shop - on or about October 22, 2020, employees performed welding operations and were required by the employer to wear tight fitting respirators without fit testing exposing them to respiratory hazards.  (d)	West Bay Maintenance Shop - on or about October 22, 2020, employees performed welding operations and were required by the employer to wear tight fitting respirators without respirator training occurring prior to use and annually thereafter.

29 CFR  1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a)	West Bay Maintenance Shop - on or about October 22, 2020, employees performed parts cleaning and welding operations with a portable eye wash unit that was not inspected, serviced or maintained which would not be suitable to flush the eye exposing employees to eye injuries.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a)	West Bay Maintenance Shop - on or about October 22, 2020, employees performed parts cleaning with a dip tank that contained Zep and did not have the other labeling as required.
