29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) On or about September 19, 2018, and at times prior, A Smith Construction, Inc., as the controlling employer, did not maintain an effective program which provided for frequent and regular inspections of the job site, materials and equipment by a competent person. Employees of Oscar Olivas-Castillo, a subcontractor, were exposed to fall hazards of up to 12 feet while conducting re-roofing activities without any fall protection systems. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
