29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    On or about September 20, 2018, on the west wall of the building, and employee was cleaning holes in the concrete blocks from a scaffold which did not have full planking, exposing him to a fall of  14'7".

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    On or about September 20, 2018, inside the building, an employee was riding on top of a pallet that was on the forks of an all terrain forklift, exposing him to a fall of  14'7".
