29 CFR 1910.180(h)(3)(i)(b): The employer did not assure that the load was well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches. On or about September 19, 2018 at Well #LHS1704BH the employer did not ensure the load, a Maximizer III pump gear box and its attached arms, was properly balanced prior to allowing employees to utilize a QMC hydraulic truck crane to hoist/lift it.

29 CFR 1910.180(h)(3)(iii)(b): During hoisting, care was not taken to ensure that the load did not contact any obstructions. On or about September 19, 2018 at Well #LHS1704BH the employer failed to ensure that the load being hoisted by a QMC hydraulic truck crane did not come in contact with an obstruction while dismantling a pumping unit.
