(a)  On or about September 14, 2018, an employee feeding cows at a dry cow facility located at E389 Luxemburg Rd. Luxemburg, WI was not provided effective information and training on the hazardous chemicals in their work area including FS-100 cleaner.     (b)  Employees exposed to hazardous chemicals such as, but not limited to, FS-100 cleaner were not provided training in the globally harmonized system labeling elements and safety data sheets.
