Pennsylvania Ave., South Charleston WV: On or about 09/21/18, employees working on ladder jack scaffolds were not protected from falling approximately 15'-16' to the ground below. Kanawha Valley Siding and Windows, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1050448, citation number 01, item number001(a) and was affirmed as a final order on 07/16/15 , with respect to a workplace located at 317 Tyler Way, Scott Depot WV 25560.
